MDO’s IN PERSON HOLIDAY PARTY 2023

BEFORE REGISTERING FOR THE MDO IN PERSON HOLIDAY PARTY YOU MUST READ THE FOLLOWING DUTY OF CARE SAFETY PRECAUTIONS AND ACKNOWLEDGE THIS DISCLAIMER

 

A) THE IN PERSON HOLIDAY PARTY IS ONLY FOR MDO MEMBERS WHOSE MEMBERSHIP IS NOT EXPIRED

-if your membership has expired and you have not been able to pay your annual fee using any of the available options

(Paypal, eTransfer, mail) we will accept cash on site

-unlike previous years, we have to restrict attendance at the party to only MDO members due to limited budget and space

 

B) EACH MEMBER MUST REGISTER INDIVIDUALLY AFTER READING THIS DOCUMENT IN FULL

 

C) MDO IS LOOKING FORWARD TO HAVING EVERYONE COME IN FROM THE COLD AND OFFER EACH MEMBER A FESTIVE MEAL INCLUDING A QUARTER CHICKEN WITH WHITE OR DARK MEAT (make selection on registration form OR vegetarian meal if required), FRIES / MASHED / BAKED, COLESLAW, DRESSING, CRANBERRY SAUCE, GRAVY, CHOCOLATES & A SOFT DRINK / TEA / COFFEE. 

ALCOHOL IS NOT PERMITTED DURING OUR EVENT.

 

D) MDO IS BEING PROVIDED THE ENTIRE FRONT RIGHT HAND SIDE OF THE DINING ROOM.  IT IS WHEELCHAIR ACCESSIBLE.  THERE IS AMPLE PARKING ON SITE AND THE RESTAURANT IS CONVENIENTLY LOCATED ACROSS FROM THE CARLINGWOOD MALL ON MAJOR BUS ROUTES.  NO ONE FROM THE GENERAL PUBLIC WILL BE IN THIS AREA FOR OUR EVENT EXCEPT STAFF OF THE RESTAURANT.
 

E) ON THE DAY OF THE PARTY (DECEMBER 5TH), WE ASK THAT EACH MEMBER SCREEN THEMSELVES FOR COLD OR FLU-LIKE SYMPTOMS (SEE BELOW) AND IF EXPERIENCING ANY PLEASE MAKE THE BEST JUDGEMENT AS TO WHETHER OR NOT TO ATTEND THE EVENT WHICH MIGHT PUT OTHERS AT RISK OF ALSO BECOMING ILL.  IF YOU ARE FEELING UNWELL OR ARE NOTICEABLY SICK, PERHAPS IT WOULD BE BETTER TO REST OR REMAIN HOME.

• Fever and/or chills

• Temperature of 37.8 degrees Celsius/100 degrees Fahrenheit or higher

• Sore throat

• Headache

• Cough or barking cough (croup) - not related to asthma, post-infectious reactive airways, COPD, or other known causes or conditions you already have

• Shortness of breath - not related to asthma or other known causes or conditions you already have

• Decrease or loss of smell or taste - not related to seasonal allergies, neurological disorders, or other known causes or conditions you already have

• For adults > 18 years or older - Unusual tiredness, lack of energy (not related to depression, insomnia, thyroid dysfunction, or other known causes or conditions you already have)

• Fatigue, lethargy, malaise and/or muscle aches/joint pain

• For children < 18 years - Nausea, vomiting and/or diarrhea not related to irritable bowel syndrome, anxiety, menstrual cramps, or other known causes or conditions you already have
   

F) WE WILL HAVE FUN, BE GRATEFUL FOR BEING TOGETHER AND WELCOME FESTIVE HOLIDAY ATTIRE!
 

By clicking the REGISTER button below, you are acknowledging that an inherent risk of exposure to illness exists in any public place where people are present.  By attending the MDO HOLIDAY PARTY, you voluntarily assume all risks related to such exposure and agree not to hold MDO or any of its affiliates, board of directors, officers, employees, agents, contractors or volunteers liable for any illness or injury.
