MDO’s IN PERSON HOLIDAY PARTY 2022

BEFORE REGISTERING FOR THE MDO IN PERSON HOLIDAY PARTY YOU MUST READ THE FOLLOWING DUTY OF CARE SAFETY PRECAUTIONS AND ACKNOWLEDGE THE COVID 19 WAIVER


A) THE IN PERSON HOLIDAY PARTY IS ONLY FOR MDO MEMBERS WHOSE MEMBERSHIP IS NOT EXPIRED

-if your membership has expired and you have not been able to pay your annual fee using any of the available options (Paypal, eTransfer, mail) we will accept cash on site

-unlike previous years, we have to restrict attendance at the party to only MDO members due to limited budget and space


B) EACH MEMBER MUST REGISTER INDIVIDUALLY AFTER READING THIS DOCUMENT IN FULL


C) MDO IS LOOKING FORWARD TO HAVING EVERYONE COME IN FROM THE COLD AND OFFER EACH MEMBER A FESTIVE MEAL INCLUDING A QUARTER CHICKEN WITH WHITE OR DARK MEAT (make selection on registration form OR vegetarian meal if required), FRIES, VEGGIES, DRESSING, CRANBERRY SAUCE, GRAVY, CHOCOLATES AND A SOFT DRINK / TEA / COFFEE. ALCOHOL IS NOT PERMITTED DURING OUR EVENT.


D) MDO IS BEING PROVIDED THE ENTIRE FRONT RIGHT HAND SIDE OF THE DINING ROOM. IT IS WHEELCHAIR ACCESSIBLE. THERE IS AMPLE PARKING ON SITE AND THE RESTAURANT IS CONVENIENTLY LOCATED ACROSS FROM THE CARLINGWOOD MALL ON MAJOR BUS ROUTES. NO ONE FROM THE GENERAL PUBLIC WILL BE IN THIS AREA FOR OUR EVENT EXCEPT STAFF OF THE RESTAURANT.


E) ON THE DAY OF THE PARTY (DECEMBER 6TH) IN ORDER TO ATTEND, EACH MEMBER MUST ANSWER “NO” TO EACH OF THE 6 SCREENING QUESTIONS BELOW:


1. Are you currently experiencing one or more of the symptoms below that are new or worsening and not related to other known causes or conditions?


•Fever and/or chills

•Temperature of 37.8 degrees Celsius/100 degrees Fahrenheit or higher

•Cough or barking cough (croup) - not related to asthma, post-infectious reactive airways, COPD, or other known causes or conditions you already have

•Shortness of breath - not related to asthma, COPD, chronic heart failure or other known causes or conditions you already have

•Decrease or loss of smell or taste - not related to seasonal allergies, nasal polyps, neurological disorders, or other known causes or conditions you already have

•Muscle aches/joint pain – not related to other known causes like for example osteoarthritis, fibromyalgia or getting a COVID-19 vaccination and/or flu shot in the last 48 hours (if you received a COVID-19 vaccination in the last 48 hours and are only experiencing mild fatigue, muscle aches, and/or joint pain that only began after vaccination, answer “No.”)

•Extreme tiredness, fatigue, lethargy, malaise and/or general feeling of being unwell, lack of energy and not related to other known causes or conditions (for example, getting a COVID-19 vaccine and/or flu shot in the last 48hrs, depression, insomnia, thyroid dysfunction, anemia, malignancy)

•Sore throat – Painful swallowing or difficulty swallowing, not related to other known causes or conditions (for example, post-nasal drip, acid reflux)

•Runny or stuffy/congested nose – Not related to other known causes or conditions such as seasonal allergies, being outside in cold weather, chronic sinusitis

•Headache – Not related to other known causes or conditions such as getting a COVID-19 vaccine and/or flu shot in the last 48 hours, tension-type headaches, chronic migraines

•Nausea, vomiting and/or diarrhea – Not related to other known causes or conditions such as transient vomiting due to anxiety, chronic vestibular dysfunction, irritable bowel syndrome, inflammatory bowel disease, side effects of medication

•Abdominal pain – Not related to other known causes or conditions such as menstrual cramps, gastroesophageal reflux disease

•Pink eye – Not related to other known causes or conditions such as blepharitis, recurrent styes


Symptoms should not be chronic or related to other known causes or conditions. The symptoms listed above are the symptoms most commonly associated with COVID19. If these symptoms are new or worsening, you should isolate and seek testing.


2. Have you been told that you should currently be quarantining, isolating or staying at home?


Could include being told by a doctor, health care provider, public health unit, federal border agent or other government authority. This can be because of an outbreak or contact tracing.


Please note there are federal requirements for individuals who travelled outside of Canada, even if exempt from quarantine.


3. In the last 10 days, have you been identified as a “close contact” of someone who currently has COVID-19?


If public health has advised you that you do not need to self-isolate, answer “No.”


4. In the last 10 days, have you tested positive for COVID-19?


This includes a positive COVID‐19 test result on a laboratory-based PCR test, rapid molecular test, rapid antigen test or other home-based self-testing kit.


5. In the last 10 days, has someone in your household (someone you live with) been identified as a “close contact” of someone who currently has COVID-19 AND advised by a doctor, healthcare provider or public health unit to self-isolate?


6. Is anyone you live with currently experiencing any new COVID-19 symptoms and/or waiting for test results after experiencing symptoms?


Current Ontario guidelines state if you had symptoms of COVID-19 in the last 10 days, do not have a fever and your symptoms have been improving for 24 hours or longer (48 hours if you had nausea, vomiting and/or diarrhea)


For 10 days after your symptoms started:


•wear a well-fitted mask in all public settings (including school and child care)

AVOID NON-ESSENTIAL ACTIVITIES where you need to take off your mask (for example, playing a wind instrument in music class or removing your mask for sports or DINING OUT)


F) UPON ARRIVAL DO NOT IMMEDIATELY GREET OTHERS WITH A HANDSHAKE OR HUG AND ASK FIRST SINCE THIS MIGHT NOT BE WELCOMED


G) CHOOSE YOUR SEAT SOON AFTER ARRIVAL SO YOUR SERVER CAN NOTE YOUR MEAL OPTION & DRINK


H) IT IS RECOMMENDED THAT MASKS BE WORN AT ALL TIMES EXCEPT WHILE EATING OR DRINKING


I) HAVE FUN, BE GRATEFUL FOR BEING TOGETHER AND WE WELCOME FESTIVE ATTIRE!

By clicking the REGISTER button below, you are acknowledging that an inherent risk of exposure to COVID-19 exists in any public place where people are present. By attending the MDO Holiday Party, you voluntarily assume all risks related to exposure to COVID-19 and agree not to hold MDO or any of its affiliates, board of directors, officers, employees, agents, contractors or volunteers liable for any illness or injury.
